Description
AMENDMENT 1 Amendment 1 purpose is to extend the closing date and post the answers to industry questions. The closing is now going from Monday, October 19, 2026 at 11:00 am ET to Tuesday, October 27, 2026 at 11:00 am ET. In the attached document section of this posting is the Answers to Industry questiond under document "Vendors questions for OH Medical exams RFQ."

- The Virginia National Guard is looking for a vendor to provide medical exams to employees who may have been exposed to hazard materials. 1. COMMUNICATION WITH THE GOVERNMENT: A. Questions: Submit written questions via email ONLY to the contract specialist Katina Moore at katina.l.moore7. [email protected]. No phone calls will be accepted. Questions will be received until October 9, 2026, no later than 11:00 a.m. Eastern Standard Time (EST).

Addendum to FAR 52.212-2(b), Submission of offers. The offeror shall submit an electronic copy of their dated quote via e-mail to: [email protected] with the solicitation number W912LQ-27-A001 in the subject block by October 19, 2026, no later than 11:00 a.m. EST. Facsimile quotes will not be accepted.
Offerors who do not provide all the required information with their quotes may be deemed NONRESPONSIVE for further consideration for the solicitation and award. In addition to the items required by 52.212-1(a), the offeror shall complete and provide the following with their submission:
- Signed and completed Information Sheet. (Attachment 1)
- Completed Reps and Certs for 52.219-1
- Proof of medical liability insurance
- Pricing Schedule attached.
