If you submit any additional bid documents where the proposed costs are inconsistent with the costs specifically proposed in Attachment A, then the cost specified in Attachment A will govern the Applicant's competitive bidding evaluation. For any Attachments insert your Name in column B1 and your SPIN in column C1. For "Facilities Owned/Leased", please indicate whether you are the owner of the transport. - SPAM and/or robotic responses not tailored to the Applicant's requirements will be deemed non-responsive and will be disqualified from consideration.
