OPTOMETRY SERVICES

STATEMENT OF WORK

Subject
The Health Services Department at the FCC Oakdale requires a contractor to provide Optometry Services as prescribed in the description of duties listed below at the Federal Correctional Complex (FCC), 1507 E. Whatley Rd, Oakdale, Louisiana.
Place of Performance
Services will be performed in the Health Services Department. Schedule
A session is defined as 1 hour. A normal workday will consist of 8 sessions/hours, 6 days a month, which will consist of a total of 48 sessions a month and 576 sessions annually. A monthly schedule will be agreed upon after award by mutual agreement of all parties. This schedule is subject to change to meet the needs of the institution. The contractor will normally work between 7:30 – 4:00 with a 30-minute unpaid lunch break. Again, this schedule is subject to some variation as the needs of the institution vary. Any change in schedule or adjustments in sessions will require prior approval by the Contracting Officer’s Representative (COR). Payment for services will only be based on scheduled sessions of work according to contract. A schedule that is mutually acceptable to the government and the contractor can be determined after award of the contract.
Due to the nature of the correctional setting, delays in performance of work may be experienced. Such delays may include but are not limited to inclement weather, electrical or mechanical problems, or other security matters. When Blanket Purchase Agreement (BPA) employees report to the institution but are unable to perform services due to such delays, the BPA employee will be paid for one session. Normally, any remaining sessions for that day will be suspended and the BPA employee will be free to leave, and the BPA employee will not be paid for the remaining unperformed sessions. However, if the institution requests that the BPA employee remain at the institution for eventual performance, the BPA employee shall be paid for such sessions. In the event of any absence, the BPA employee may make up his/her time so long as it does not exceed 6 days in any given month. If the BPA employee fails to provide coverage, the Government may seek a BPA remedy.
Education and Experience
The contractor will provide FCC Oakdale with an optometrist who possesses a current and valid optometric license/certification to practice optometry in the United States. The optometrist must possess a current ACLS certification. The contractor must provide verification of credentials
