INITIAL CHECKLIST FOR CONTRACTOR PERSONNEL

In accordance with Attachment 01 Statement of Work (SOW), as an authorized representative of the above listed contractor, with my signature below, I do hereby certify that each of the following SOW Section 3.3.18 requirements are submitted for the abovelisted contract employee. Check off each of the below indicating that the SOW Section 3.3.18 requirement is complete and provided to the FAA.

Certification of medical standards and current drug test completion and date (Section 3.3.9 and Section J Attachments 13 and 14). (armed)

State of New Jersey valid Firearms permit (armed)

3.

Completed Contract Security Officer Firearms Qualification Course of Fire (CoF) and Certification Form (Sections 3.1.1, 3.2.1, 3.2.2, 3.3.5, and 3.5.6 and Section J Attachment 09). (armed)

4.

Certification of U.S. citizenship (Sections 3.2.1, 3.2.2, 3.3.5, and 3.3.7 and Section J Attachment 12). (all)

6.

Certification of minimum age (Section 3.2.1, 3.2.2, 3.3.5, and 3.3.7 and Section J Attachment 12). (all)

Certification of written and oral communication skills (Section 3.8 and Section J Attachment 12) (all)

Applicable state or local jurisdiction licensing (Section 3.3.11). (armed)

Security Officers Registration Act (SORA) certification (Sections 3.2.1, 3.2.2, and

American Red Cross or American Heart Association-approved Cardiopulmonary Resuscitation (CPR) certification. first aid, and Automated External defibrillator (AED) certification (Section 3.6.1). (armed)
