SECTION B – SUPPLIES OR SERVICES & PRICES/COSTS
B.1 DESCRIPTION
The Department of Homeland Security (DHS), Federal Emergency Management Agency (FEMA), Region IX Pacific Territories, specifically Guam, American Samoa, and the Commonwealth of the Northern Mariana Islands (CNMI) has a requirement for Annual Medical Membership Services. The Contractor shall provide emergency medical membership/access, 24/7 assistance center access, clinical intake, initial triage, medical case management, mission coordination, commercial medical escort coordination, ground ambulance coordination, fixed-wing air ambulance coordination and/or performance, specialty medical transport team support, specialty medical equipment, companion travel coordination, legal referral coordination, readiness validation, reporting, and related support services for covered FEMA personnel on official work-related assignment in specific Region IX Pacific Territories.
B.2 CONTRACT TYPE
This is a single award Indefinite Quantity Indefinite Delivery (IDIQ) contract, individual task orders may issued under this contract with Firm Fixed Price (FFP), Time-and-Material (T&M), and Cost Reimbursable (CR) line items based on the requirements of the task order. Task Orders shall be issued in accordance with section G.4 entitled “Task Order Procedures”. The Government shall not issue, and the Contractor shall not accept Task Orders under this IDIQ Contract for the procurement of services that are not within scope of the requirement as clearly described in Section C.
B.3 MINIMUM AND MAXIMUM AMOUNT
The guaranteed minimum for all contracts awarded under this solicitation is $12,500.00. The minimum Guarantee is subject to availability of funds per FAR 52.232-18. The Not to Exceed ceiling for all contracts awarded under this solicitation is $2.1 million inclusive of the base period and all option periods. With respect to the maximum quantity of services, the total amount of services ordered under this IDIQ contract shall not exceed the total productive capacity of all vendors under this contract.
B.4 SERVICE CONTRACT ACT WAGE DETERMINATIONS
