Conduct Concrete and Civil Site Repairs RFI’S.
1-I have a workers’ compensation waiver, but I am bidding a VA construction project in Colorado. I need a letter confirming whether Great White Construction LLC has an EMR assigned. If no EMR exists because of the waiver or insufficient payroll history, please state that on agency or carrier letterhead. I also need to know what workers’ compensation/employer liability coverage I would need if I send employees to Colorado.
If your company does not have enough records for 3 years of EMR or OSHA report just send a letter justifying that.
2-Concrete Repair Method: Please confirm whether concrete repair locations identified on the repair maps may be completed by full removal and replacement in lieu of slab jacking/leveling where applicable.
Generally, the leveling repair methodology has been left to the contractor to decide, with Attachment B Repair List stating the VA’s expected use conditions for the warranty period. 3-Sink/Depressed Area: Please clarify the intended extent of repair for the sink/depressed area, specifically whether the Contractor is expected to excavate unsuitable material to competent subgrade or whether a defined excavation depth is required.
The Government does not expect any excavation, just a fill of the sink/depressed area and surface restoration.
4-Material Testing: Please confirm whether Contractor-provided third-party material testing is required and, if so, the required testing frequency for concrete and compacted fill. The Government will not require material testing for this project. 5-Can you verify that there are requirements for a full time onsite supervisor and a dedicated SSHO for the Concrete and Civil Site Repair project?
The superintendent, SSHO, and QC may all be the same individual. 6-Section 01 00 00 – General Requirements: This section is referenced on page 7 of the solicitation but does not appear among the posted documents. Could the Government please provide this specification section?
See revised specifications set with 01 00 00 included.
7-Map 32 – Self-Compacting Fill: Please confirm the approved self-compacting fill material for the two sink pits shown on Map 32, including any applicable strength, gradation, compaction, or performance requirements.
See new Attachment F.
8-Attachment C – Precast Concrete Planters: Please confirm whether an equivalent product to the specified Petersen P96X30X36 planter is acceptable. If so, please provide the applicable equivalency and
