For a nurse practitioner or physician assistant proposed as Key Personnel, must the Tennessee collaborating physician agreement be executed and submitted with the quote? Or is it acceptable to identify the collaborating physician and show evidence the agreement will be in place before performance begins? The latter. Under SOW 3.2.2, if no physician is proposed, does the Government expect the NP’s or PA’s collaborating physician to provide any on-site time at the Center, or does compliance with Tennessee’s protocol and chart review requirements satisfy this task? The latter will suffice. Will the provider dispense or administer controlled substances from Center stock, or only prescribe? They will only prescribe If the Center is an additional prescribing location in Tennessee. Is a Tennessee DEA registration at the provider’s primary practice address acceptable? Yes Attachment 2 references FAR 52.228-5 and AGAR 452.228-71, and SOW 7.1.5 mentions general liability insurance. What general liability coverage amounts are required in addition to the $1 million medical liability coverage, and must proof be submitted with the quote? 1 million liability coverage is adequate. Each individual provider’s name must be on the policy. We need a copy of the policy. Will Key Personnel require a PIV card or fingerprinting under FAR 52.204-9 / AGAR 452.204-71 before starting? No PIV card but we do need a background check completed. If an NP or PA is proposed, must their Tennessee collaborating physician also be identified as Key Personnel and submit a resume, license, DEA registration and insurance with the quote? We need the collaborating agreement only. May offerors submit a retyped copy of Attachment 2, the Quoter’s Questionnaire, with all items and answers, in place of the original form? Yes, as long it it contains the required information. SOW 7.1.1 requires Key Personnel to “have an active, unrestricted license to practice independently in the state where services will be provided.” Because Jacobs Creek is a federally operated Forest Service facility: (a) Would the Government accept a nurse practitioner with an active, unrestricted Virginia APRN license and national board certification (FNP-BC), but no Tennessee license? No, since they are not a federal employee, they must be licensed in the state of the center: i.e. Key Personnel must hold a Tennessee license.
