12444526Q0082, Eddyville Quarters Window Replacement questions:
Insurance: The Statement of Requirement (solicitation page 1) says the Contractor shall provide all labor, supervision, specialized tools, protective gear, transport vehicles, insurance, and construction materials. No insurance clause (such as FAR 52.228-5), coverage types, minimum limits or certificate requirement is included. Does this require the prime contractor itself to carry insurance or furnish its own certificate of insurance, or is it satisfied by the general liability, workers' compensation and auto insurance carried by the installing subcontractor that will perform all on-site work? If specific coverages or limits are required, please state them. This is satisfied by the general liability, workers' compensation and auto insurance carried by the installing subcontractor that will perform all on-site work
Past performance: Factor 2 (solicitation page 15) evaluates the offeror's record through SAM.gov exclusions and federal performance databases, and the optional Experience Questionnaire (Attachment 5) asks about experience as a prime contractor and/or subcontractor. Will the experience and past performance of our proposed installing subcontractor be considered in the evaluation, for example if documented on the Experience Questionnaire?
All information submitted for Past Performance will be considered in the determination. The form included in the package is an optional format that includes the information requested..
Lead-based paint: The Statement of Work (Attachment 1) says the residence has been in service since the early years of the Shawnee National Forest, and the Project Specifications (Attachment 7) note that the quarters are occupied and that existing trim is to be removed and reinstalled. The solicitation addresses only asbestos. (a) Was the house built before 1978, and has it been inspected or tested for lead-based paint? If so, can the results be shared? (b) If lead-based paint is present or presumed and EPA's Renovation, Repair and Painting (RRP) rule applies, will the Forest Service accept the installing subcontractor's EPA RRP firm certification and its certified renovator, or must the prime contractor also hold its own EPA RRP firm certification? If the prime must hold it, by what date (quote submission or start of work)?
The building was constructed and altered in the 1930’s thru the 1950’s. While it has not been tested, it is assumed because of its age that lead based paint is present.
