SUBJECT: Offeror Past Performance Assessment in Support of Request for Proposal (RFP) Number W519TC-26-R-A076

Dear Sir / Ma’am,
The US Army Contracting Command – Rock Island is currently conducting a competitive source selection to evaluate Offerors on the subject RFP. As part of this evaluation, we have requested that the Offerors provide information about their past performance on recent and relevant federal, state, or local Government and/or commercial contracts involving services performed under North American Industry Classification System (NAICS) 541612 or similar efforts. You have been identified as the point of contact for the enclosed past performance reference.

Your assessment of their performance is extremely valuable to our evaluation. The Government will consider the information provided in this questionnaire together with information obtained from other sources when evaluating the Offeror’s past performance. The Government will assign an overall Past Performance rating of Acceptable or Unacceptable based on whether the Government has a reasonable expectation that the Offeror will successfully perform the requirements of the solicitation. The questionnaire requests your assessment and supporting rationale; however, you are not being asked to assign the Government’s final Past Performance rating. Please provide factual, complete, and candid information based on firsthand knowledge of the contractor’s performance. If you do not have sufficient knowledge to provide a meaningful assessment, please identify that limitation in the questionnaire.

Please complete and return the enclosed questionnaire to the Contracting Officer at email: [email protected] and Contract Specialist at email [email protected], no later than fifteen (15) calendar days after receipt of request or DATE, whichever occurs first.
