PAST PERFORMANCE INFORMATION SHEET

Instructions

This Instruction Sheet describes the information to be provided when submitting contracts for your Past Performance Volume. You will provide the information requested in this form for each contract/program being described. Comments shall be frank and concise regarding your performance on the contracts you identify. Provide a separate completed form for each contract/program submitted. There is no need to reformat the Submittal Sheets; they should be submitted as is and without changes in color, font, text boxes, tables, etc. You do not need to submit this Instruction Sheet with your Submittal Sheets. You only need to submit the information found on the Submittal pages 3-4 for each submitted contract. Limit the number of past efforts submitted and the length of each submission to the limitations set forth in paragraph in Section L Instruction to Offerors paragraph 4.4. The Submittal Sheets currently have two pages, but as information is entered in the text boxes, the pages can expand to the limits found in Section L. Past performance submissions for any team member will be evaluated for relevancy based on how similar work on the prior contract coincides with the requirements of this acquisition as outlined in Section M, Evaluation Factors for Award. Relevancy will only be determined based on the role the team member submitting the contract Past Performance Information Sheet will play on this acquisition.

The following are instructions related to the sections found on pages 3-4:

A. Offeror: Provide information for your company. For large, multi-functional companies, limit the references to work done by the division, group or unit that plans to perform the proposed work. If the company or division performing this effort is different than the offeror or the relevance of this effort to the instant acquisition is impacted by any company/corporate organizational change, note those changes. Refer to the “Organizational Structure Change History” you provide as part of your Past Performance Volume.

B. Program Title: Title used to describe the contract/effort.

C. Contract Specifics:

1. Contracting Agency or Customer: The party who awarded the contract. 2. Contract Number: If information is provided relevant to task order type contracts, offerors must provide specific task order numbers, in addition to the basic contract number. 3. Contract Role refers to your role on the contract (i.e. Prime Contractor or Subcontractor) and any comments you feel are necessary to clarify that role (no comments are necessary).
