AACCEPTABLE - During the contract period, contractor performance is meeting (or met) all contract requirements. For any problems encountered, contractor took effective corrective action.

UUNACCEPTABLE- During the contract period, contractor performance is failing (or fail) to meet most contract requirements. Serious problems encountered, corrective actions were either ineffective or non-existent. Extensive Customer oversight and involvement was required.

NANOT APPLICABLE – Unable to provide a score. Performance in this area is not applicable to effort assessed.

UTA Unable To assess - The individual completing this questionnaire does not possess the background or knowledge to accurately answer. (e.g. Contracting Officer answering technical questions)
